Temporal Fossa Augmentation


The temporal fossa is located above your cheekbones, just to the side of your eyes. It is the shallow depression of your skull.

You know it as your temple. Created by an underlying pair of cranial or temporal bones, it houses the temporalis muscle, one of the strong muscles necessary for chewing. If you take away the muscle, you are left with a space called the temporal fossa. Normally, this indentation is a discreet hollow just below and behind your forehead. But if the depression is so pronounced that it makes you look sickly or gaunt, you may want your surgeon to fill it in. Augmenting a temporal fossa depression isn’t a necessary step in feminizing your face, but it will add immensely to a healthy, feminized look. 

There is no difference between the sexes when it comes to an exaggerated temporal fossa depression. Even though I have treated only one woman with a significant indentation, I believe strongly that a deeply concave or inwardly curved space has nothing to do with gender. Whether you are a man or a woman, a pronounced hollow just behind your lateral orbital rims, the bony ridges to the side of your eyes, can have a significant effect on your facial aesthetics. This hollow can make you appear starving when you are well fed. Although the majority of the people don’t have a temporal fossa depression that merits attention, it is not an uncommon occurrence.

The degree and precise location, however, can be variable. The depression also isn’t necessarily symmetrical or the same on both sides. When it does exist, augmenting the space can offer many aesthetic benefits. Improving the contour can create a more appealing look.

Mandibular Angle Reduction (Reduction Angleplasty)


Mandibular Angle Reduction is usually done to individuals who have wide jaw line or square face. It reduces the width of the lower face and change a square face to an oval one.

This procedure can enhance the masculine features, in men while in women it can give an impression of strength and confidence. In some, however, the jaw can be disproportionately wide as compared to the rest of the face or zygomas and mandibular angles are overly prominent that upsets the balance of the face.

This is one reason for aesthetic Mandible Angle Reduction. The incision is commonly done inside the mouth which means the scar will be invisible.

Lower Jaw Tapering- Angle Reduction/ V-line mandibular Angle Contouring


Tapering the mandible or lower jaw is an important step in facial feminization surgery. Women have more tapered and narrower lower face. Men have considerably prominent jaw line rendering a wide, square face which enhances the masculine features. With these differences, the surgery is essential in transitioning from male to female. Thus, improving femininity and eliminating obvious masculinity.


It allows for reduction of the chin to a more slender and oval shape without increasing its length. The surgery also reduces the overly strong bone flanking each side of the chin, thus transforming the more masculine U shape to a sharper and more feminine V shape. The chin line is excised into a smooth line. The chin end can be brought together without touch of the nerve. The protruding parts in both sides of the mandible are excised. The narrowed chin end produces the wholly oval V-line face type.

Chin Reduction


The chin is one of the most important features in gender identification. It is so different between the genders. In men, chins are vertically higher and wider than in women. The chin is the front portion of the mandible or lower jaw and the point on the lower face.

Chin reduction is a cosmetic surgical procedure that reduces the size of the chin by shaving down the bone through an incision made in the lower gums. Chin reduction surgery begins with the surgeon making an incision inside the mouth (near the lower lip) or under the chin. Using specialized instruments, the surgeon detaches the bone at the tip of the chin and then reshapes and removes the excess bone to properly proportion the contour. The tip is then repositioned with the remaining section of chin and held together using wires and plates. The incisions are sutured to complete the procedure.

Malar Bone Reduction/ Cheek Bone Contouring


Cheek Bone Reduction is not a common procedure in transgender individuals among Caucasians, Hispanics or African Americans whose wide cheek structure is regarded as very attractive while in Asian individuals this is a very popular procedure since an oval-shaped face is considered to be more aesthetically pleasing and desirable.


Reduction Malarplasty is performed on patients with large cheek bones by either of two techniques. The first involves using a special drill to burr down or shave the excess bone at the zygomatic arch, the bony ridge that extends from beneath the eye socket along side the skull lateral to the temporal fossa. The second involves cutting out an appropriate amount of the zygomatic anterior arch, rotating or turning the lateral arch inward and securing with a small plate and two tiny screws. Both approaches maybe use depending on the problem.

Forehead Augmentation (Forehead Contouring Surgery)


You can easily tell the difference between male foreheads from female ones. Covering a large portion of the face, the forehead shows a markedly different contour and projection in men than in females, although most people don’t realize how their forehead is shaped or even protrudes from the face.

Scalp Reduction/ Scalp advancement


The foreheads and scalps are facial features that show more clearly whether a person is a male or a female. Male foreheads are remarkably different from female ones. Naturally, the distance from the hairline down to the eyebrow at the mid pupil line in males averages 2.6 inches in contrast to about 2 inches in women.

High hairlines and vertically long foreheads are generally accepted in males but not fashionable in females, therefore scalp reduction is performed as part of facial feminization surgery.

With scalp reduction, the portion of the scalp area is removed then pulled together, thereby shortening the forehead and minimizing the receding hairline. The best candidates for this procedure are individuals who suffer from male pattern balding and those who wanted to look more like a woman.

Lateral Eyebrow Lift

Lateral Eyebrow Lift corrects the drooping of the outer brow line. This technique is also known as a temporal brow lift, and addresses sagging around the outside of the eyebrow almost exclusively. Lateral brow lifts can be performed in conjunction with blepharoplasty or other procedures.
During a lateral brow lift, incisions are placed in the hairline at the temples towards the sides of the face. The lateral brow lift removes excess skin, lifting the outer corners of the forehead. Unlike a traditional brow lift, a lateral brow lift will not raise the middle of the brow, but it will somewhat alleviate crow's feet (the creasing at the outer corners of the eyes). Lateral brow lifts are ideally suited to prospective patients who are only concerned with the drooping of the ends of the eyebrow.

Duration of the operation:
The procedure normally takes about 20-30 minutes.

Post operative care:
When your procedure is completed, your forehead may be taped and your head may be loosely wrapped with a soft compressive dressing usually for 1-2 days to minimize swelling and bruising. A thin tube may be present to drain any excess blood or fluid that may collect under the skin. You may experience little or no postoperative pain for the first few days. Analgesic medications for pain and antibiotics to prevent infection will be prescribed.

Recovery Period:
Swelling in the eyelids can be profound but is usually short lived. Most patients can go back to work in about a week. Sutures or staples maybe remove at 5-7 days post op.

Possible risk and complications:
Complications are rare; however, there are risks associated with any medical procedure. Some patients may not be able to move their eyebrows or forehead. Scar formation and permanent hair loss within the scar area may occur. Permanent lack of sensation at the incision line is also a rare complication. Infection, bleeding, and other minor complications are possible.

Anesthesia use:
Most brow lifts are performed under local anesthesia with sedation.

Sunglasses and protective sun block are required for several weeks. Lip Reduction, also called reduction cheiloplasty, is the procedure to remove excess lip tissue to reduce the appearance of overly large lips. 
Lip reduction is performed by making an incision in the length of your lip inside your mouth and then removing a strip of skin from one or both lips, depending upon the look you want to achieve. Your Surgeon will aim to enhance the mouth and lips area to complement your facial features, giving you a softened, natural mouth.

Duration of the operation:
The procedure takes only about 30 minutes - 1 hour.

Post operative care:
You may notice a change in your smile, odd sensations of “hollowness”, tingling, the sporadic sharp pain, or “pulling”, burning, and cold sensation. Take it easy and sleep on two pillows to keep your head elevated for around 7-14 days or how long your surgeon suggests. Remember that swelling and bruising id perfectly normal after surgery but this will clear up after a few weeks.
